    Planning for your future

    Some of the laws we talked about earlier require that schools help you plan for your future before you even leave high school.

    What is an IEP?

    Under IDEA, the services that you get to support you were determined by something called an Individualized Education Plan or IEP. An IEP is also required in Part 200 of NYS law. If you are eligible for services under IDEA, you have to have an IEP in place.


    For years now, your teachers and your school have been getting together on at least a yearly basis to talk about what you needed to be successful at school.


    ·       Did you need time with a teacher to focus on reading?
    ·       Did you need to work with a therapist to improve the way that you can move around in school?
    ·       Did you need extra time on tests?

    All of this is determined by your IEP. You can think of it as a contract between you, your school, and your parents to make sure that you get an education that works for you.

    What is Transition Planning?

    These laws also guarantee that you can get assistance to move into the next part of your life. This process is called transition planning. Transition planning is the process of making a plan to support you as you prepare to leave school and begin your adult life. Transition planning is designed to help you make sure that you can plan for a future that is right for you.

    Transition planning is an outcome oriented process, which means that you have to have a long-term goal or outcome identified that you will work towards from the time you start transition planning until you leave school.

    Each year, you will identify goals for yourself that help you work toward that long-term goal that you have identified.

    When does transition planning start?

    According to NYS law, the transition planning process should begin with your first IEP, when you are 15. However, if your school thinks it will help, they can start the process even earlier. This is something that you and your parents can request to give you an advantage as you think about what you might like to do after high school.

    In this chapter, we will talk about what the transition process is supposed to help you do, what the meeting should look like, and the role that you and your parents should play in this planning.

    Goals and Planning

    The transition planning process is designed to help you to achieve your goals for life after high school. It should be outcome-oriented. So everything that you do around transition planning should move you towards a life-long goal.


    Think about the process that you went through when you did your person-centered planning meeting – all things led to that dream or long-term goal you had for after school. Your IEP during transition planning should do the same thing.

    Who creates an IEP?

    The IEP is created by a team of people at your school. When transition planning starts you are officially required to be part of that team. Other team member should include:

    • Special education teacher
    • General education teacher
    • School administrator
    • Your parents

    Also, you and your parents can invite others to sit at the table if you think that they will be able to help. Others who may be invited include a transition specialist, your PROMISE case manager, your job coach or other adult service system representative, or a therapist or counselor that you have been working with in school.

    What is the purpose of an IEP?

    The IEP is a tool to help you plan for your future. It helps to guide the courses you take in school. It also should clearly identify any support needs that you have to reach your goals and clearly address how the school will support you to meet those needs.


    The IEP should clearly describe what services the school is providing to you – it is really a contract between you, the school, and your parents to make sure that you are getting the resources and support you need to reach your goals for the future. In New York, you continue to use your IEP as your transition plan. After you turn 15, it should start to focus on developing skills and having goals that are for after you get out of school.

    504 Plan

    Section 504 of the Rehabilitation Act requires that schools provide students with disabilities who are covered under this law, but not IDEA, have a 504 plan in place. A 504 plan is a plan that the school uses for a student who needs to do something differently at school to be successful (called an accommodation), but does not need "specialized instruction". This means that you take the same tests and read the same books that everyone else in your class does (general education or special education), but you just get might get at that material in a different way. A 504 plan is really about making sure that a student has equal access to what the school teaches. This plan should be updated about once a year to make sure the accommodations that you get are working for you.

    Schools are allowed to use the same form and process for a 504 plan that they do for transition planning. Developing the IEP during transition is a multi-step process that is repeated at least once a year. Remember that the process requires that you and your family have input.

    There are five steps in the transition planning process. This includes:

    1. Assessment

    2. Goal development

    3. Incorporation of assessment into the IEP document

    4. Identification of needs, objectives, and services

    5. Implementation.

    What is an IEP for?  To make sure you get the support you need to be successful and to help you plan for your future.

     

    What is a 504 plan?  Some kids have IEP’s and some have 504 plans.  They really both do the same thing… they make sure that the kids get what they need to be successful.
